What is infertility?

Infertility is a condition affecting the reproductive system in which pregnancy has not been achieved after 12 months or more of regular, unprotected sexual intercourse.

Infertility may be related to female factors, male factors, a combination of factors, or sometimes no clear cause can be identified. Female factors may involve ovulation, the ovaries, fallopian tubes, uterus or hormonal systems.

An infertility evaluation helps your doctor understand your individual circumstances and determine whether further investigations or treatment may be appropriate.

When Should You Seek Medical Care?

Trying Without Success

If pregnancy has not occurred after 12 months of regular, unprotected intercourse, an infertility evaluation may be appropriate.

Age 35 or Older

Women aged 35 or above may benefit from evaluation after around six months of trying to conceive.

Irregular Periods

Very irregular, infrequent or absent periods may indicate an ovulation or hormonal problem and may warrant earlier assessment.

Previous Pelvic Conditions

A history of endometriosis, fibroids, pelvic infection, tubal problems or reproductive surgery may justify earlier evaluation.

Previous Pregnancy Problems

Previous pregnancy loss or difficulty conceiving after an earlier pregnancy may require an individualized fertility assessment.

Possible Male Fertility Factor

When applicable, both partners should be assessed because fertility problems can affect either partner.

Benefits of Infertility Evaluation

Not every fibroid requires treatment. Small fibroids that cause no symptoms may simply be monitored. When treatment is needed, the appropriate option depends on factors such as symptoms, fibroid size and location, age and whether future pregnancy is desired.

Identifying Possible Causes

A structured assessment can help identify medical, hormonal, reproductive or anatomical factors that may be affecting conception.

Understanding Ovulation

Menstrual history and appropriate investigations can help determine whether ovulation may be occurring normally.

Assessing the Reproductive System

Where clinically indicated, your doctor may recommend imaging or other investigations to assess the uterus, ovaries and fallopian tubes.

Evaluating Both Partners

When a male partner is contributing sperm, evaluation of both partners can help identify male and female factors rather than focusing only on the woman.

Personalized Treatment Planning

The findings from the evaluation can help your doctor decide which treatment, further investigation or specialist referral may be appropriate.

Earlier Appropriate Care

Identifying a potential fertility problem early can help patients understand their options and avoid unnecessary delays in appropriate care.

Frequently Asked Questions

What is infertility?

Infertility is generally defined as failure to achieve pregnancy after 12 months or more of regular, unprotected sexual intercourse. It may result from female factors, male factors, combined factors or sometimes remain unexplained.

When should I have an infertility evaluation?

Women under 35 without an obvious fertility problem are generally evaluated after 12 months of trying. Evaluation may be appropriate after around six months for women aged 35 or older, while women over 40 may benefit from more immediate assessment. Earlier evaluation is appropriate when there is a known condition associated with infertility.

Can PCOS affect fertility?

Yes. PCOS can be associated with ovulatory problems and may affect the ability to conceive. An appropriate evaluation can help determine whether ovulation or another factor may be contributing.

Can fibroids affect fertility?

Some uterine fibroids can affect reproductive health depending on their size and location. Your gynecologist can assess whether fibroids may be relevant to your fertility concerns.

Can endometriosis cause infertility?

Endometriosis can affect fertility in some women. If endometriosis is suspected or already diagnosed, your doctor may recommend an individualized fertility assessment.

Does infertility only affect women?

No. Fertility problems can affect women, men, both partners or sometimes have no identifiable cause. When applicable, evaluation of both partners should be considered.

What tests are done for infertility?

There is no single test for infertility. Depending on your circumstances, evaluation may include medical and reproductive history, assessment of ovulation, ultrasound, assessment of the uterus and fallopian tubes, selected laboratory tests and semen analysis where applicable.

Do I need an infertility evaluation if my periods are regular?

Regular periods are reassuring but do not rule out every possible fertility factor. If pregnancy has not occurred within the appropriate timeframe, an evaluation may still be recommended.

Can infertility be treated?

Many causes of infertility can be managed or treated, but the appropriate approach depends on the underlying cause, age, medical history and findings from the evaluation. Your gynecologist can discuss appropriate options or specialist referral where required.
